MISSED APPOINTMENT POLICY

Effective January 23 2012

All missed appointments will be billed for the
FULL COST of the appointment.

Adequate notice MUST be given to cancel an appointment:

Chiro/Physio  – 2 Hours
Massage        – 12 Hours


There is an answering machine to take messages after hours:

*All patients will be required to leave a Credit Card number on file
*Patients privileges will be suspended until account is paid in full
*All fees due at time of service
